
The Opportunity
North by Northeast Community Health Center is seeking a part-time Primary Care Physician who will bring strong clinical expertise and a commitment to patient-centered, culturally specific care in a collaborative and supportive team environment.
North by Northeast Community Health Center (NxNE) is the only medical clinic in Oregon devoted to African American/Black health. We exist to improve health outcomes and advance health equity by offering primary care services and health education and promotion focused on the African American/Black community. Approximately 95% of our patients are covered by the Oregon Health Plan (Medicaid), and the remainder are uninsured. We offer adult primary care services and are well known for our innovative community programs.
This Primary Care Physician will be dedicated to delivering high-quality, compassionate care, with a focus on patient satisfaction, continuous quality improvement, and effective care coordination. With an open and inclusive approach, they will actively engage with colleagues, patients, and the broader community to ensure care is responsive to patient needs and reflective of best practices in primary care. They will contribute to a culture where all team members feel valued and supported, fostering a sense of belonging across the organization. They will be driven by a desire to address the health inequities that impact the African American/Black community in Portland. They will embrace and actively advance North by Northeast’s mission as both a safety net for underserved populations and a model for delivering culturally responsive, high-quality care.
The position is .5–.75 FTE (negotiable), with a mutually-agreeable set schedule that includes a consistent presence in the clinic 3–4 weekdays per week. Our practice model is built around high-quality care rather than volume. North by Northeast patient visits are 30 minutes, ensuring time for meaningful connection and comprehensive care.

North by Northeast currently employs a staff of 10, which includes a clinical team made up of a Medical Director, Nurse Practitioner, Nurse Clinic Manager, Medical Assistant, and Receptionist, and an administrative/community team made up of the Executive Director, Community Care Director, Development Director, Operations Manager, and Assistance Coordinator. North by Northeast has an annual budget of $1.8 million and uses Epic as its Electronic Health Record.

Key Responsibilities
This position reports to the Medical Director. Key responsibilities include:
- Provide outpatient care to a panel of adult primary care patients in keeping with North by Northeast standards and model of care.
- Conduct ongoing health care and clinical management of stable, chronically ill patients.
- Formulate treatment plans, including medication prescriptions, based on available clinical data, and work cooperatively with patients to implement treatment plan.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical problems.
- Manage minor and/or acute illnesses and initiate treatment.
- Follow up on patients’ Emergency Department visits, Urgent Care visits and hospitalizations.
- Practice management, including but not limited to records review, phone calls and consultations.
- Accurately and comprehensively document all patient encounters in Epic in a timely manner.
- Manage Epic In-Basket according to NxNE’s timelines and procedures.
- As needed, provide backup in the absence of Medical Director, including lab results, refill requests and other patient needs.
- Adhere to state, federal and NxNE regulations, including HIPAA.
Qualifications, Knowledge, & Skills
- Physician (MD or DO) with an unrestricted license to practice in the state of Oregon (or the ability to obtain one)
- Board eligible or Board certified in a primary care-focused specialty with a training emphasis on the care of adult patients in the ambulatory setting
- Experience in:
- Outpatient management of common medical conditions such as hypertension, diabetes and depression/anxiety as well as preventive medicine
- Community, public and/or population health
- Serving medically underserved populations
- Commitment to anti-racism personally and professionally
Compensation & Benefits
This is a part-time (.5–.75 FTE) position with an agreed-upon consistent schedule that includes 3–4 weekdays on site. The annual full-time salary range for this role is $215,000–$250,000. The offered salary will be prorated based on FTE and commensurate with the successful candidate’s licensure, skills and experience. In addition, medical student loan repayment is available to eligible physicians.
